- New Client RegistrationYour contact and billing details, plus the basics about each horse and where it is kept. Please complete it before your first appointment.
- Credit Card AuthorizationWe require a card on file for every client. Payment is due at the time of service.
- Pre-Purchase / Lease Information RequestDetails about the horse, the buyer and seller, and the horse’s history, so we can plan a pre-purchase or lease exam.Coming soon
- Gastroscopy ConsentYour consent for a gastroscopy, with the fasting instructions for the day before and the morning of the procedure.Coming soon
